My circumstances have changed

Any changes to your circumstances should be confirmed in writing to DMGT Pensions.

Examples of changes we would like to hear about are:

Change of marital status (please forward a copy of your marriage certificate).
Change of surname (please forward copies official documentation – marriage certificate or Deed Poll certificate).
Change of address (please complete the change of address form)
Change of nominated beneficiary (please complete an expression of wish for lump sum death benefits form and forward it to DMGT Pensions).
Informing us of an adult dependant (please complete an expression of wish for dependant’s pension benefits form and forward it to DMGT Pensions).

If you do not complete a form, the trustees will not be able to take your wishes in to account when reaching a decision as to whom any benefits are paid.
